Fresno Adult Day Health Care Facility is a community based day program that provides health, therapeutic and social services designed to service the special needs of those at risk of placement in a nursing home.  The program emphasizes partnership with the participant family, physician and community while working towards maintaining independence in a community setting.   Fresno Adult Day Health Care provides a stimulating, carefully supervised environment, which is structured to meet the unique needs of older adults.  A medically supervised day-care assistance program includes: Administrator Staff Physician Registered Nurse Program Director Social Worker Activity Coordinator Occupational Therapist Speech Therapist Licensed Clinical Social Worker Dietician Pharmacist     The facility is in a pleasantly quiet location, free of distractions with ample parking and unimpeded entry to building. Facility Hours: Monday - Friday 8:00AM to 5:00PM Services Provided: 9:00AM to 3:00PM   Fresno Adult Day Health Care Facility offers direct services to participants by accredited or licensed professionals or Para-professionals supervised by the licensed professional.  The following basic will be provided in accordance with state regulations. Rehabilitation services, including speed therapy, physical therapy, and occupational therapy. Medical services by staff and personal physician Nursing Services Nutritional Services Social Work Services Psychiatric or Psychological Services Planned Recreational and Social Activities Transportation Services Dietician Services Hot Lunch and Snacks Services Coordinated for with an outside provider when indicated. Podiatry services provided by a podiatrist.
